Epic Systems Corporation generously awarded a $100,000 grant to assist Hartford HealthCare’s Neighborhood Health with advancing health, safety, and sustained engagement among underserved and underinsured patients.

This investment will fund chronic disease management supplies, including blood pressure cuffs and glucometer kits, needed to manage common but serious conditions within underserved populations—such as hypertension and diabetes.

Additionally, the grant will support pharmacy and transportation vouchers to help patients obtain essential medications, hygiene supplies, and care items that advance healing and well-being outside of clinical settings.
