The Connecticut Health and Educational Facilities Authority (CHEFA) generously awarded a $75,000 grant to Hartford HealthCare’s Neighborhood Health to support the program’s efforts to remove barriers to healthcare access, expand its reach and breadth of services, and help patients better manage their health from home.
“We are exceptionally grateful to CHEFA for this support,” said David Santoro, Vice President, Business Operations for Hartford HealthCare Integrated Care Partners (ICP). “The Hartford HealthCare Neighborhood Health mission is to provide quality healthcare to those greatest in need within the communities we serve. This award will help our team in providing thousands of people with better access to medical, pharmaceutical, and food services.”
“We are pleased to support Hartford HealthCare’s Neighborhood Health mobile clinics,” said Betty Sugerman Weintraub, CHEFA’s Manager of Grant Programs and Philanthropic Outreach. “It is so important for healthcare to be accessible and meet folks where they are, and this initiative accomplishes this effort by setting up mobile clinics in partnership with neighborhood organizations. CHEFA is proud to provide $75,000 to invest in the health and well-being of Connecticut residents.”
Launched by Hartford HealthCare in 2021, Neighborhood Health collaborates with community partners throughout the state to bring health experts where they are needed most and increase healthcare access for high-risk, vulnerable, uninsured, and underinsured populations and small employer groups.
The program’s mobile health clinics—which include ground teams consisting of advanced practice providers, registered nurses, medical assistants, care managers, registrars, and public safety officers—eliminate logistical barriers to traditional forms of healthcare, providing high-quality, equitable, and accessible care to residents close to home. They offer a variety of health services—from screenings and vaccinations to mental health counseling, medical referrals, education, and support—and help reconnect patients to regular healthcare providers.
